Passport Page Addition Requirements |

To add pages to your passport, provide the following:
- Your valid passport.
- The completed and signed Passport Additional Pages form (To return to Atlas web site, close screen after printing the application).
- A copy of the airline ticket, or your travel itinerary printed on travel agent letterhead.
- The completed and signed authorization form (to get the right authorization form, must choose the desired processing time).
- A check in the amount of $60.00, payable to the U.S. Department of The State.
