Passport Services

New Passport Requirements:
If you have never before applied for a passport, or if it is more than 15 years from the issue date of your existing passport, the following procedure applies to you (Important Note: The Passport Agency requires that we submit executed/signed applications within FIVE (5) days of signing. Please Keep this in mind when sending your signed and dated applications for processing):

A. Prepare these documents:
  1. A certified copy of birth certificate. Hospital records are NOT acceptable.
  2. Two recently taken passport-type identical photos.
  3. Complete a New Passport application. But, DO NOT sign the application yet.(To return to Atlas web site, close screen after you print the application)
  4. A cashier's check or money order for $170 ($140 for minors) payable to "US Department of State." (Note: Effective 02/01/08 applicants may also apply for a "PASSPORT CARD." If applying at the same time with your passport, just mark the corresponding box on application and add $30 to the filing fees. You may also use renewal application to apply for "PASSPORT CARD" separately. In this case, the filing fee for Passport Card is $90 ($75 for minors) plus $25 local processing fee (Acceptance Facility Fee) and our service fee depending on the type of process time you request
    5. Important: Post offices (Acceptance Facility) and Courthouses will charge $25 for their local processing fee. This is perfectly fine and is a part of the federal passport processing fee.
  5. Include a photocopy of front and back of your current (valid) State I.D., or driver's license.
  6. A copy of the airline ticket, or travel itinerary issued on a travel agent letterhead.
  7. If you have had a name change, include a certified copy of marriage certificate, or certified copy of court order/decree.
  8. Complete and sign the authorization form. This form allows us to pick up your new passport from the Passport Agency and must be included in the same-package with your application.
    Important: The appropriate authorization form is related to the number of processing days you select.
